  • Employee Accident/Incident Form - To report employee (not student) accidents and incidents. Employee completes top section and principal/director completes the bottom section. Send complete forms within 48 hours of the incident to Sue Maybee in Purchasing (Admin, Buidling, 425 837-7070).
